Hospital Resources
Donation provides so many benefits to your patients and others
in the community. We thank you and your hospital for your support
in facilitating the donation process. Your role in connecting
Legacy of Life with potential donor families at the time of
death is a crucial part of a very important process. THANK YOU!
Suggested Procedures:
* At the time of death, please activate the donor hotline specific
to your region or you may contact Legacy of Life at 1-800-397-3077.
This number is answered 24 hours per day, every day of the year.
* When you call, please have the following information available:
- Name of deceased
- Age of deceased
- Cause of death
- Name of next of kin
- A phone number where we can reach the next
of kin
* Given time is of the essence, please provide
as accurate information as possible, particularly relating to
a correct number where we can reach the next of kin in a timely
manner.
* We suggest you DO NOT try to discuss the
option of donation with the family. We have found a telephone
call from our Family Services Coordinator is more comfortable
for the family and more effective at explaining the option of
donation.
* If it is convenient, we would ask you to
hand out a copy of the LEGACY OF LIFE brochure to the family.
You can also mention that Legacy of Life will be contacting
the family. Again, we suggest leaving the donation "presentation"
to the staff of Legacy of Life. We will provide you copies of
this brochure, but you can download, by clicking
here (5MB).
Compliance Assistance:
As you probably know, there is a legal requirement that you
contact a donation organization after any death in your hospital.
We will do everything we can to assist you with compliance,
but we also hope you work closely with us for a better reason:
it is the right thing to do for many people. Remember, each
donor can touch the lives of over 100 recipients, making donation
one of the mos t meaningful contributions anyone can make.
Representatives of Legacy of Life will meet with your hospital
to help you develop a comprehensive compliance program, as well
as reporting and documentation process to prove your compliance
should you ever be questioned on such things. View us as your
consulting partner through this process.
We can also provide your hospital with compliance reports designed
to meet the requirements of the Joint Commission.
In-Service Education
Legacy of Life provides training to your hospital staff through
in-services. In addition to typical training and review of processes,
we also take some time to recognize and thank you and your staff
for your contribution to the donation process.
These in-services also serve to document your hospital's compliance
with the donation regulations.
Bereavement Assistance to Families
In addition to contacting families regarding donation, our staff
will assist families in coping with their loss. We provide a
number of group meeting opportunities, a series of booklets
mailed to assist the family with grieving, and multiple opportunities
to honor donors and their families. We genuinely believe donation
can be an important part of the healing process for families
and we work with them to achieve this end.
Communications Tools
To assist with your communications, we have developed a number
of tools to facilitate communications regarding donation. Many
can be downloaded and printed directly from this website.
* A Brochure for Donor Families. Contact
Legacy of Life to obtain copies of this brochure. Click
here to download a generic copy of this brochure if you wish
to print some at your location. (5MB)
* Legacy Poster - An 18 x 24 poster
which can be displayed in areas where potential donor families
may be exposed to it.
* Hospital Training Guide - printed
copies of our hospital training is provided to our our partner
hospitals, however, you can download a copy of this training
here. (coming soon)
* The Legacy of Life website - please
feel free to direct potential donor families to this website.
There is an extensive "Family Resources" section of the website.

The donation legacy…
A 63 year old grandmother is diagnosed with severe
peripheral vascular disease, which threatens the
continued use of her leg. Faced with amputation,
the woman chose the implantation of a saphenous
vein from a donor. The resulting successful surgery
gived her the opportunity for an active life for
many years to come. |

The donation legacy…
A father’s back pain becomes so debilitating it threatens not only his ability to work, but also his ability to actively interact with his family. An orthopedic spinal fusion surgery, aided with allograft bone tissue from a donor, returns the patient to an active lifestyle and allows him to live his life as he always had. |

The donation legacy…
A star volleyball player’s world changes with one awkward fall which severely damages her knee. Doctors predict a stronger recovery by reconstructing the knee with ligament tissues from a donor. The successful surgery enables the patient to accept a scholarship to play volleyball at the collegiate level. |
